• Individual support: Standing alongside clients as they navigate complex systems such as housing, healthcare, benefits, and legal resources.
• Justice-impacted individuals: Helping people with criminal justice involvement overcome barriers to housing, employment, and services by connecting them to fair opportunities and supportive programs.
• People experiencing homelessness: Elevating the needs and voices of unhoused neighbors, working to secure safe shelter, stable housing, and access to essential services.
• Veterans: Supporting veterans in accessing the benefits, housing, and mental health resources they have earned, and advocating for trauma-informed, culturally competent care.
• Low-income individuals and families: Addressing systemic barriers to stability by connecting clients with income supports, rental assistance, and community resources.

Navigating healthcare, behavioral health, and social services can be overwhelming. Our care coordination team helps connect participants to the care they need.

Each participant works with a case manager who provides ongoing, one-on-one support. We focus on the goals that matter most to the individual.

Case management may include:

• Goal setting and individualized service planning
• Assistance with IDs, benefits, and documentation
• Support with transportation, appointments, and referrals
• Advocacy with systems and providers

We can help with:

• Primary care and urgent care connections
• Mental health and substance use treatment referrals
• Support with insurance, benefits, and paperwork
• Coordination between providers and follow-up support

We assist participants in finding and maintaining safe housing. Our staff work with local shelters, housing programs, and landlords to identify options that fit each person’s situation.

We can help with:

• Referrals to emergency shelters and interim housing
• Applications for supportive and permanent housing programs
• Coordinated Entry and housing assessments (if relevant in your area)
• Problem-solving and support to maintain housing
• Our on-site emergency housing provides short-term, immediate shelter for individuals in crisis who need a safe place to stay. While in emergency housing, clients receive basic necessities and support while staff work with them to identify longer-term housing options and connect them to case management, benefits, and other needed services.
