Navigating healthcare, behavioral health, and social services can be overwhelming. Our care coordination team helps connect participants to the care they need.

Each participant works with a case manager who provides ongoing, one-on-one support. We focus on the goals that matter most to the individual.

  • Support with insurance, benefits, and paperwork
  • Coordination between providers and follow-up support

We assist participants in finding and maintaining safe housing. Our staff work with local shelters, housing programs, and landlords to identify options that fit each person’s situation.


We can help with:

  • Referrals to emergency shelters and interim housing
  • Applications for supportive and permanent housing programs
  • Coordinated Entry and housing

  • Coordinated Entry and housing assessments (if relevant in your area)
  • Problem-solving and support to maintain housing
  • Our on-site emergency housing provides short-term, immediate shelter for individuals in crisis who need a safe place to stay. While in emergency housing, clients receive basic necessities and support while staff work with them to identify longer-term housing options and connect them to case management, benefits, and other needed services.
